ALL EMPLOYEES AND VOLUNTEERS MUST HAVE CURRENT CLEARANCES
There are always on-going training sessions in the local area. Please refer to the parish bulletin or see the archdiocesan website for current locations and times.

CRIMINAL BACKGROUND AND CHILD ABUSE CLEARANCES
1. Waiver Release Form (permission for Resurrection Parish to process)
2. Criminal Background Check Form (one for Volunteers and one for Staff)
3. Child Abuse Form
4. FBI – Fingerprinting (ALL EMPLOYEES)
5. Disclosure Statement Application Form (ALL VOLUNTEERS)
Criminal background check, child abuse clearances and FBI fingerprinting must be current or renewed every 5 years.
FINGERPRINTING FOR VOLUNTEERS IS ONLY REQUIRED IF YOU HAVE NOT LIVED IN THE STATE OF PENNSYLVANIA IN THE LAST 10 YEARS.

Register on line at www.pa.cogentid.com or call 1.888.439.2486
Select the Pennsylvania Department of Education (PDE) Service.
Parish employees register under the Department of Human Services.
There will be a fee when registering.
DISCLOSURE STATEMENT APPLICATION FORM FOR VOLUNTEERS (DSAF)
ALL VOLUNTEERS ARE REQUIRED by The Child Protective Service Law to have on file a completed and signed DSAF document or FBI clearance.
Archdiocesan Policy Classes and Certificates
All employees/volunteers must be in compliance with the Archdiocesan Safe Environment Policies.
This includes training and signed certificates in both Standards in Ministerial Behavior and Mandated Reporter.
Standards in Ministerial Behavior Training: Applicants register on line through www.virtus.org. Follow the prompts for location and time of classes being offered in your area.
Mandated Reporter Training: This training is offered on line only through the (OCYP) Office for Child and Youth Protection Training Institute @ www.childyouthprotection.org.
Please bring all completed and signed forms and certificates to the Rectory Office to the attention of Joan Stulz.
